FOR SINGLE PROPRIETORSHIP/MICRO-BUSINESS/SMEs/INDIVIDUAL

1. Completely accomplished:
   1. Customer Information Sheet
   2. Buyer’s Acknowledgement
   3. Purchase Agreement
2. Photocopy of any Two (2) Valid IDs of Individual or Authorized Representative and Dependents (verified with original):
   1. Driver’s License
   2. SSS ID
   3. PRC ID
   4. Passport
   5. Company ID
   6. Postal ID
   7. BIR / TIN ID
   8. Voter’s ID
3. Business Permit, DTI Registration, and BIR Registration (if applicable)
4. Two (2) copies of most recent 2x2 ID Photo of principal member and dependents or extensions
5. IF AVAILING OF FINANCING / INSTALLMENT PLAN:
   1. Latest Proof of Billing (not more than 3 months) – Original preferred. Photocopies accepted subject to verification.
   2. Any One (1) of the following
      1. ITR
      2. Audited Financial Statement
      3. Latest 2 months Bank or Credit Card Statement

FOR CORPORATIONS/PARTNERSHIPS/NGOs/FOUNDATIONS WITH SEC/COOPERATIVES WITH CDA

1. Completely accomplished:
   1. Corporate Information Sheet
   2. Buyer’s Acknowledgement
   3. Purchase Agreement
2. SEC registration for Corporations, Partnerships, NGOs, Foundations or CDA Certificate for Cooperatives
3. Proof of Authority to transact – Corporate Secretary’s Certificate or Letter of Authorization issued by the Company’s highest ranking officer
4. Photocopy of any Two (2) Valid IDs of Authorized Representative and Assignees (verified with original):
   1. Driver’s License
   2. SSS ID
   3. PRC ID
   4. Passport
   5. Company ID
   6. Postal ID
   7. BIR / TIN ID
   8. Voter’s ID
5. Two (2) copies of most recent 2x2 ID Photo of principal member and assignees or extensions
6. IF AVAILING OF FINANCING / INSTALLMENT PLAN:
   1. Latest Proof of Billing of company’s utility bills (not more than 3 months) - Original preferred. Photocopies accepted subject to verification.
   2. ITR or Audited Financial Statement or Latest two (2) months Bank Statement or Certificate of Paid-up capital for pre-operating companies (Copy of certification issued to SEC)

FOR LGUs AND GOVERNMENT AGENCIES

1. Completely accomplished:
   1. Corporate Information Sheet
   2. Buyer’s Acknowledgement
   3. Purchase Agreement
2. Photocopy of any Two (2) Valid IDs of Authorized Representative and Assignees (verified with original):
   1. Driver’s License

3. Oath of Office of Authorized Representative
4. Two (2) copies of most recent 2x2 ID Photo of principal member and assignees or extensions

   1. Latest Proof of Billing of LGUs utility bills (not more than 2 months) - Original preferred. Photocopies accepted subject to verification.
   2. Proof of Budget Allocation or Fund Availability under the name of Widus International Leisure Inc.
