b. POEA contract or contract from employers.
c. Notarized or authenticated Special Power of Attorney by the consul authorizing a person up to 2nd Degree of affinity or consangunity (e.g. parents, siblings, spouse, children, parents-in-law, sister/brother-in-law) based in the Philippines to sign documents on his behalf.
d. Proof of remittance for the past 12 months.
2. Non Residents Filipino and Foreigners with co-borrower living in the Philippines
a. The same documents in letter a above nos 1 and 2.
(The co-borrower is up to the 2nd degree of consanguinity)
II. IDENTITY
A. Philippine Residents
1. Photocopy of any valid ID, any two - SSS/GSIS card, Passport, Driver's License, NBI Clearance, Marriage Contract and Office ID.
2. Two (2) ID Pictures 1 1/2 x 1 1/2.
3. Photocopy Community Tax Certificate.
4. Tax Account Number (TAN/TIN).
B. Non Philippine Residents
1. Photocopy of Passport or any valid ID showing picture and borrower's signature.
2. Two (2) ID Pictures 1 1/2 x 1 1/2.
3. Tax Account Number (TAN/TIN).
